The Hospital identified a census of 18 inpatients and no current Swing Bed patients. Twenty three medical records were chosen for review including five Swing Bed closed records. Based on document review and staff interview, the Hospital failed to inform 5 of 5 Swing Bed patients reviewed of all of their rights (patients #19, 20,21, 22, 23).
Findings included:
- Review of the document titled "Rights/Responsibility #0 R6/08", and the pamphlet titled "Patient Rights" provided to all patients, revealed the Hospital failed to inform the patients of their right to have access to persons and services inside and outside the facility.
Interview with Swing Bed staff member A and Licensed staff member B, on 4/20/10 at 12:00pm verified the two documents were the only information provided to the Swing Bed patients.

Based on document review, record review, and staff interview, the Hospital failed to include 5 of 5 Swing Bed patients reviewed with the right to participate in planning their plan of care(patients #19, 20 ,21 ,22, 23).
Findings included:
- Review of the Hospital policy for "Swing Bed Status", date effective 10/06, stated the "...Patient's Rights"...6... "You also have the right to participate in planning your care..."
Review of the medical records for swing bed patients #19, 20, 21, 22 and 23 failed to evidence patient participation in their plan of care.
Swing Bed staff member A, on 4/20/10 at 11:35am stated Hospital staff conducted swing bed patient care planning meetings every week, but patient's were not invited.
Licensed staff member B, on 4/20/10 at 12:03pm verified that the Hospital failed to invite any Swing Bed patients to participate in their plan of care.

Based on document review and staff interview, the Hospital failed to inform 5 of 5 Swing Bed patients reviewed of all of their rights (patients #19, 20, 21, 22, 23).

Based on document review and staff interview, the Hospital failed to inform 5 of 5 Swing Bed patients reviewed of all of their rights (patients #19, 20, 21, 22, 23).

Based on document review and staff interview, the Hospital failed to inform 5 of 5 Swing Bed patients reviewed of all of their rights (patients #19, 20, 21, 22, 23).
Findings included:
- Review of the document titled "Rights/Responsibility #0 R6/08", and the pamphlet titled "Patient Rights" provided to all patients, revealed the Hospital failed to inform the patients of their right to share a room with their spouse when married and in the same facility, when both spouses agree.
Interview with Swing Bed staff member A and Licensed staff member B, on 4/20/10 at 12:00pm verified the two documents mentioned above were the only information provided to the Swing Bed patients.

The Hospital identified a census of 18 inpatients and no current Swing Bed patients. Twenty three medical records were chosen for review including five Swing Bed closed records. Based on document review, record review, and staff interview, the Hospital failed to provide 5 of 5 Swing Bed patients reviewed with an ongoing program of activities (patients #19, 20, 21, 22, 23).
Findings included:
- Review of the medical records for the five sampled Swing Bed patients revealed no documented activity assessment and lack of documented activities offered or participated in by any of the patients.
Interview with Swing Bed staff member A, on 4/20/10 at 11:35am, revealed the Hospital had no activities planned for the Swing Bed patients and stated they believed nursing performed the Activity Assessment.
Interview with Licensed staff member B, on 4/20/10 at 11:28am and 12:03 pm revealed the Hospital previously employed an Activities Coordinator, but failed to have anyone in that position for the past 1-2 years. Staff member B revealed that activity assessments were not completed on the five sampled patients and stated no staff has completed this since the Activity Coordinator left.
Interview with Licensed staff members C and D, on 4/20/10 at 2:05pm verified the Hospital lacked a policy directing staff on the completion of an activity assessments and ongoing activities for the Swing Bed patients.
