Based on interview and document review, the hospital failed to provide active screening of staff for COVID-19 prior to each shift of work, in compliance with CMS guidance. This failure had the potential to impact all patients at the hospital.

Findings include:

On 4/30/20, at 10:15 a.m. administrative nurse executive (ANE)-A was interviewed and stated the hospital was not screening staff for signs and symptoms of COVID-19 before each shift. ANE-A stated staff were trained related to when they should not come to work, and staff were monitoring themselves.

On 4/30/20, at 1:00 p.m. employee health registered nurse (RN)-G stated the hospital was not conducting employee health screening prior to each shift. RN-G stated if a staff member provided care for a COVID-19 patient, he or she would be asked to self monitor for 2 weeks and check in via email.

The facility had no current staff screening policy.
