Based on observation of surveyor 16732 on August 24, 2011


Patient room door number 205 will not latch properly.

Based on observation of surveyor 16732 on August 24, 2011

1) OR corridor has combustible storage in corridor reducing egress width to 6 feet.

2) OR corridor has cart for supplies store across from PIXIS machine reducing corridor to 5 feet.

3) OR Corridor has computer on wheels and table stored with clothing for personal reducing corridor width to 5 feet.

4) Emergency room corridor has computer on wheels with separate chair infront of computer reducing the hallway width to 5 feet.

5) Large linen cart stored in corridor turning into ER hallway reducing width of corridor.

Based on record review of surveyor 16732 on August 24, 2011:

The fire alarm system is not being tested and maintained in accordance with NFPA 72, National Alarm Code as there has not been any testing of the fire alarm system within the last year.

Based on observation of surveyor 16732 on August 24, 2011:

1) Combustible storage in central supply room within 5 feet of cylinders.

2) The door for the central supply room was unlocked so unauthorized persons could enter.

Off site buildings surveyed pursuant to the National Fire Protection Association 101 Life Safety Code 101 2006 Edition as referenced by MRSA Title 25 Section 2452 based on the occupancy use. Based on observation of surveyor 16732 on August 24, 2011

Family Medicine Building emergency light does not work in West wing.

Based on observation of the surveyor 16732 on August 24, 2011, the following National Electrical Code violations were noted:

1) New electrical panels in the kitchen have carts stored infront of them within 36 inches from the panel.

2) In storage room of kitchen combustible storage stored under electrical panel within 36 inches of panel.

3) Boiler 1 in furnace room has missing cover on junction box above it.

4) Boiler 2 in furnace room has cover off pressure control box.

5) Boiler 3 in furnace room has missing cover off junction box above furnace.

6) Missing cover on junction box at ceiling in purchasing room above cart that has surgical drapes stored

7) Missing cover on junction box in air handler room on roof.

8) Power relocatable tap being used to plug in refrigerator at break room in OR area.
