Based on observation and interview, the facility failed to properly clean debris from high surfaces throughout the facility. This deficient practice has the potential to result in unsanitary conditions affecting all patients. Findings include:
On June 7, 2021 at around 9:30 AM during a tour of the emergency department, dust accumulation was observed on most of the sloped top wall cabinets, high sections of the mast arm of the examination lights mainly at the fast-track section, and at top of the blanket warmers cabinets and the pyxis units located inside the medication rooms serving the sections of the emergency department, and at the central nurses' station wall mounted cabinets.
On June 7, 2021 at around 12:40 PM during a tour of the main tower dust accumulation was observed on high surfaces at the lab, radiology, cardiac vascular recovery area of the first floor, at the inpatient rehabilitation gym section of the 6th floor, and the clean utility rooms of the 3rd and 4th floors west wings.
On June 7, 2021 at around 3:15 PM the environmental services manager was interviewed. The environmental services manager confirmed that high surfaces cleaning is required throughout the facility. It was stated that "the environmental services department is currently experiencing staff shortages and that is one contributing factor to the lack of proper high surfaces cleaning". All above findings were also confirmed by all accompanying staff members FF, GG, and HH at the time of the survey.
