Based on observation and interview, it was determined the facility failed to maintain exiting requirements in three instances throughout the facility.
Findings Include:
Observation on 12/14/10 revealed the following:
a) At 10:25am, there was an exit door in the anesthesia corridor that was being locked with a combination lock.
b) At 10:42am, the same day surgery unit on the second floor did not meet the requirements of a suite. The facility was directing people to exit through the unit.
c) At 11:00am, there were two delayed egress doors in an egress path on the fourth floor maternity unit.
Interview with the facility representative confirmed the condition existed on the time and date stated above.

Based on observation and interview, it was determined the facility failed to maintain sprinkler requirements in two instances throughout the facility.
Findings Include:
Observation on 12/14/10 revealed the following:
a) At 8:17am, the alcove area on the third floor near ICU waiting room lacked sprinkler protection. The compartment is considered fully sprinklered.
b) At 10:00am, there were sprinkler hangers being supported by other sprinkler pipes in the main sprinkler room on the first floor.
Interview with the facility representative confirmed the condition existed on the time and date stated above.

Findings Include:
Observation on 12/13/10 and 12/14/10 revealed the following:
a) On 12/13/10 at 10:56am, there were unsecured carbon dioxide cylinders in the first floor kitchen storage room.
b) On 12/14/10 at 9:10am, there was an unsecured helium cylinder in the loading dock gas storage area.
Interview with the facility representative confirmed the condition existed on the time and date stated above.

There was temporary wiring running through the ceiling supporting computer equipment in the main first floor computer room.
