Based on review of hospital policies and procedures, medical record reviews, facility document and video surveillance reviews, and staff interviews, the hospital failed to have an effective nursing service providing oversight of day to day operations for safe nursing care by failing to ensure implementation of post fall procedures for 2 of 4 post fall patients reviewed (Patients #2, #5).
