FINDINGS INCLUDE:
On facility tour between 8:00 AM and 2:00 PM on 03/09/2015, observation revealed that:
There was no automatic smoke detector with sounder base present in the M.D. Resident Sleeping Room (A341).
This deficient practice was confirmed by the Director of Maintenance upon discovery.

Medical gas storage and administration areas shall be protected in accordance with NFPA 99, Standard for Health Care Facilities.
(a) Oxygen storage locations of greater than 3,000 cu.ft. are enclosed by a one-hour separation.
(b) Locations for supply systems of greater than 3,000 cu.ft. are vented to the outside.

FINDINGS INCLUDE:
On facility tour between 8:00 AM and 2:00 PM on 03/09/2015, observation revealed that:
4 oxygen cylinders were observed in an upright position unsecured by chain or other securing device in the oxygen storage room in the Home-OPBH Office/Clinic.
This deficient practice was confirmed by the Director of Maintenance upon discovery.
