Based on observation, it was determined that the facility did not maintain an environment to ensure acceptable level of safety and quality. Specifically, the facility did not maintain the main mattresses in the two operating rooms (OR) so that they could be sanitized.

Findings include:

On the morning of 1/14/2014, a tour of the facility's OR's was conducted. The top of the main mattress of the operating table in OR 1 was observed to have a 1/4 inch tear exposing the padding underneath to liquid chemicals and rendering it unsanitizable. OR 2's main mattress located on the operating table was observed to have a 1 inch tear on top exposing the padding to chemicals and rendering it unsanitizable.

Based on personnel file review and interview it was determined that the Governing Body did not reappoint and credential 1 out of 12 individual practioners/allied health staff to the medical staff, even though they continue to work for the hospital. (Staff identifier: 11)

Findings include:

On 1/15/14, twelve medical staff/allied health staff personnel records were reviewed.

Medical staff 11's personnel files were reviewed. The last documented appointment and credentials on file expired on 1/31/12.

A "Provider List January 13, 2014" was reviewed. Medical Staff 11 was listed as an "Associate" on the document.

On 1/15/14, an employee from human resources was interviewed. It was stated that Medical Staff 11 regularly works in the emergency department. It was stated that the hospital's Medical Staff had been debating for over 2 years whether to re-credential and reappoint Medical Staff 11. Medical staff 11 was licensed as an allied health worker. Since the hospital has numerous physicians appointed, the medical staff has not been able to decide if they need or want Medical Staff 11's services.
