Based on observation and interview, the facility does not ensure that all means of egress are maintained, free of obstructions at Buffalo General Medical Center (BGMC).
Findings include:
Observation on 11/14/19 at 01:30 PM at BGMC lower level AL-186 Radiology revealed an egress corridor and door to the stair tower were obstructed. The door had two wood pallets directly against it. Adjacent to the pallets, there were two platform carts, one with empty cardboard boxes on it and the other with many full boxes, labeled "Sterile Disposable Syringes".

Based on observation and interview, the facility does not ensure all exit doors are unlocked at Millard Fillmore Suburban Hospital (MFSH).
Findings include:
Observation on 11/19/19 at 11:00 AM in the MFSH mechanical room (above 2-North), a door leading to the outside, with an exit sign above it, was locked. The door required a key to unlock it.

Based on observation and interview, the facility does not ensure dead-end corridors do not exceed 30 feet at the Oishei Children's Hospital (OCH).
Findings include:
Observation on 11/15/19 at 11:00 AM on the OCH 9th, 11th and 12th floors revealed exit signage from the lobby area leading to only one exit. This created a dead-end corridor greater than 100 feet.

Based on observation and interview, the facility does not ensure that sprinkler heads within a compartment are of the same thermal sensitivity and are not obstructed at Millard Fillmore Suburban Hospital (MFSH). This creates the potential for sprinkler heads not to be able to activate in a fire or to have the spray pattern obstructed.
Findings include:
Observation on 11/22/19 at 10:30 AM in the MFSH Box Picker area of the pharmacy revealed three quick response sprinkler heads, five standard response sprinkler heads, and two upright sprinkler heads with metal fusible links, all in the same compartment. Additionally in the pharmacy, a sprinkler head was located three-inches from the top of a metal cabinet.

Based on observation and interview, the facility does not ensure all vertical openings were properly enclosed at Oishei Children's Hospital (OCH).
Findings include:
Observation on 11/15/19 at 09:00 AM in the OCH 7th floor conference room revealed a penetration in the floor, which continued through the deck to the ceiling of the 6th floor. This penetration was to accommodate electrical receptacles, and IT connections.

Based on document review and interview, the facility does not ensure all staff participate in fire drills at least quarterly.
Findings include:
Review on 11/15/19 at 01:00 PM of fire drill documentation at Buffalo General Medical Center (BGMC) revealed that for the 2nd quarter of 2019, the "Kaleida Health Fire Drill Evaluation Report" indicated 30 staff participated in fire drills. For the 3rd quarter, the "Kaleida Health Fire Drill Evaluation Report" indicated that 44 staff participated in fire drills, and the "Life Safety Training" document indicated 18 staff participated in fire drills.
Interview on 11/15/19 with Staff #1, Manager of Regulatory Affairs, confirmed that there are 2,500 staff at BGMC.
Review on 11/19/19 at 08:30 AM of the fire drill documentation at Millard Fillmore Suburban Hospital (MFSH) revealed for the 2nd quarter of 2019, 324 of 1,600 staff participated in fire drills. No documentation was submitted to DOH staff for 3rd quarter 2019.
Interview on 11/19/19 with Staff #3, VP of Quality & Patient Safety confirmed MFSH has 1,600 staff.

Based on observation, and interview, the facility does not ensure electrical receptacles are maintained.
Findings include:
Observation on 11/13/19 8:45 AM to 03:30 PM at Buffalo General Medical Center revealed an electrical receptacle, located in the hallway across from bay 26 on the 6th floor, an electrical receptacle GFCI located in room #27 on the 6th floor, GI bay #13 on the 5th floor, and an electrical receptacle in (red) room #5 on the fifth floor, were loose and not secured. The test button was out and would not reset on an electrical receptacle GFCI in room #50 on 6th floor. An electrical receptacle (red) in room #6 on the 4th floor had a metal prong from a plug broken off in one of the contact openings.

Observation on 11/14/2019 at 02:45 PM at Oishei Children's Hospital revealed the electrical receptacle (white) at the head of the bed in room J-1121, and an electrical receptacle located in 2LCL2-44 X-Ray room #4, were loose and not secure.
