Based on document review and interview, the facility failed to ensure that the Emergency Department (ED) Central Log contained information regarding each patient that comes to the Emergency Department (ED) for care.

Findings included:

On 07/28/2022, at approximately 10:19 am, during interview with Staff K, ED Registrar, staff stated the following: Patient #1 came with his wife on 4/17/22 with complaints of chest pain. The patient and wife waited for some time before departing the ED. The patient said that they were going to another hospital.

Review of the Emergency Department log did not reveal any evidence that Patient #1 was in the ED on 4/17/22.

During interview with Staff J, ED Registrar on 7/28/22 at 10:01 am, the staff stated that the registration information for Patient #1 was not in the system because the patient left. The information was taken out by the Charge Nurse. Staff stated "if a patient was not triaged, we do not save their information."

Review of the policy titled "Emergency Department Log" (Reviewed: April 2021) did not indicate that each patient presenting to the ED would be entered into the ED central log.

On 07/29/2022, during the exit conference, these findings were acknowledged by Staff A (RN, ED Assistant Director), Staff G (Senior Director of Patient Relations), Staff S (RN, AVP Performance Improvement and Risk Management), and Staff T (MD, Chief Medical Officer).
