The inspector observed, while accompanied by the CEO/Administrator during the hours of the inspection from 2:00 pm to 4:00 pm on 11/9/2016 that there were the following issues. They were: In the main entry, the fire alarm pull box was approximately 20 feet from the exit door. A fire alarm pull box must be within five feet of each emergency exit door.

They were: In the kitchen, the minimum clearance of 18 inches was not being maintained between stored items on storage racks and the sprinkler heads.

The inspector observed, while accompanied by the Plant Operations Manager during the hours of the inspection from 9:30 am to 12:00 pm on 11/9/2016 that there were the following issues. They were: In the outdoor medical gas tank storage area, the medical gas tanks were not adequately secured. They need to be provided with racks, chains, or other fastenings to individually secure all cylinders, whether connected, unconnected, full, or empty, from falling.

They were: There electrical panel boxes in the corridor that were not locked. They must be locked to prevent unauthorized tampering.

They were: The electrical room was used for storing parts and containers of chemicals. A clear path of at least 30 inches must be maintained in front of the electrical panels.

Electrical Systems - Receptacles

The inspector observed, while accompanied by the Plant Operations Manager during the hours of the inspection from 9:30 am to 12:00 pm on 11/9/2016 that there were the following issues. They were: The critical care receptacles (red) did not have permanent labels indicating panel and circuit number. All critical care area receptacles shall be identified. The face plate for the receptacle(s) shall have a non-removable label or be engraved indicating the panel and circuit number. Here, non-removable means that it cannot be peeled off with a fingernail.

Gas Equipment - Cylinder and Container Storag

They were: The repertory room contained supplies, desks, and a supply oxygen tanks. Oxygen is highly combustible. These tanks must be stored in a storage location that is specifically designed to safely store medical gas tanks.
