Through observation during the survey, November 2, 2012 , it was determined that the facility failed to maintain the doors to the corridor.

During the walk through of the facility, with the Maintenance Director, resident room #407 was found to not positively latch into its frame.

During the walk through of the facility, with the Maintenance Director, a mobile linen cart was found in storage in the corridor by room# 400.

Through observation during the survey, November 2, 2012, it was determined that the facility failed to install and maintain the electrical system in accordance with NFPA 70.

During the walk through of the facility, Maintenance Director, an open electrical box with loose wiring was found at the exit door by room # 400.
