Based on record review and interview, the facility failed to ensure timely notification of the patient's family member of admission. The deficient practice is evidenced by failure to notify a patient's wife of admission for more than 2 1/2 days after admission in 1 (#1) of 3 (#1-#3) reviewed patient records.
Findings:
Review of the medical record for Patient #1 revealed admission on 03/26/2024 at 7:40 p.m. On admission the patient signed the release of information to allow his wife to participate in his care. Further review revealed the patient's wife was not contacted until 03/29/2024 at 12:16 p.m. for collateral information.
In interview on 05/06/2024 at 9:32 a.m., S2QD verified the wife of Patient #1 was not contacted until several days after admission.

Based on observation and interview, the hospital failed to ensure care in a safe setting. The deficient practice is evidenced by staff allowing patients to use pointed objects during art therapy. Findings:
Tour of the facility on 05/06/2024 between 1:15 p.m. and 1: 30 p.m. revealed a watercolor paint brush with a wooden handle on the cabinetry. Further observation revealed a patient using fine felt tipped pens.
In interview at the time of discovery, S2QD verified the items could be used as weapons and were prohibited items.

Based on record review and interview, the facility failed to ensure drugs and biologicals were administered according to the physician's order. The deficient practice is evidenced by failure of the nursing staff to administer medication as ordered for elevated blood pressure.
Findings:
Review of the orders for Patient #1 revealed an order initiated on 03/27/2024 at 1:00 a.m. for "clonidine HCL 0.1 milligram; One tablet by mouth every four hours PRN for Systolic >160; Diastolic >90." The order was discontinued 04/02/2024 at 10:42 a.m.
Review of the vital signs revealed the following elevated blood pressures:
On 03/27/2024 at 7:15 a.m., the patient's blood pressure was 148/96 mmHg.
On 03/ 28/2024 at 9:15 a.m., the patient's blood pressure was 147/95 mmHg.
On 03/29/2024 at 6:44 p.m., the patient's blood pressure was 155/101 mmHg.
Review of the Medication Administration Record for the above dates and times revealed the medication was not administered.
In interview on 05/06/2024 at 10:05 a.m., S2QD verified the medication was not administered as ordered.
