Based on observation and staff interview, the facility failed to maintain corridor separation walls by allowing an unsealed penetration. This deficient practice would allow smoke and hot gases to migrate into the exit corridors. The facility is licensed for 24 beds, with a census of 2 on the day of survey.
Findings are:
Observation on 10-8-19 at 12:01 pm revealed a 2 inch hole in the corridor wall above the door to the Administrative office.
During an interview on 10-8-19 at 12:01 pm, Maintenance Staff A and B confirmed the findings.

Based on observation and interview, the facility failed to prohibit the use of extension cords and power strips as a substitute for adequate wiring. This deficient practice would create electrical injury and increase the chances of a fire. The facility has the capacity for 24 beds with a census of 2 on the day of survey.
Findings are:
Observation on 10-8-19 between 12:44 pm and 12:55 pm revealed the following:
1. A multi-strip fastened to the wall with duct tape below the desk in the central nurse's station.
2. The daisy chaining of one multi-strip into another multi-strip and into an extension cord beneath the front reception area desk and countertop.
During interviews on 10-8-19 between 12:44 pm and 12:55 pm, Maintenance Staff A and B confirmed the findings.
