Based on observation and interview, the facility failed to ensure clear signage was posted in the X-Ray and Computed Tomography (CT) machine locations, identifying hazardous radiation areas. This deficient practice had the potential to affect all patients and staff utilizing the radiation services. Findings include:
During an observation on 10/26/21 at 11:10 a.m., the CT room was located behind one door that opened into the main corridor of the hospital. Above the door was a sign that turned on which indicated "in use." There was no signage posted to identify a hazardous radiation area.
During an observation on 10/26/21 at 11:13 a.m., the door which entered into the X-Ray room did not have signage to identify a hazardous radiation area.
During an interview on 10/26/21 at 11:13 a.m., staff member H stated she did not remember ever having signs posted on the X-Ray and CT rooms to identify a hazardous radiation area. Staff member H stated the "in use" signs were turned on to indicate when the X-Ray and CT machines were being used.
During an observation on 10/27/21 at 7:35 a.m., no hazardous radiation area signage was posted on the doors of the X-Ray or the CT room.
During an interview on 10/27/21 at 9:44 a.m., staff member B stated she was not aware of when or if radiation signage was ever posted on the CT or X-Ray rooms, to identify hazardous radiation areas.
