Based on observation and interview, it was determined the facility failed to ensure expired sutures were not available for patient use in two (#1 and #2) of four (#1-#4) ED (Emergency Department) rooms. The failed practice did not ensure patients were receiving only currently dated supplies and created the potential to affect any patient needing sutures in the ED. Findings follow.
A. During a tour of the ED Room #1 on 07/19/16 at 1130, one of four boxes 6-0 Nylon Suture, was observed to have expired on 01/16
B. During a tour of the ED Room #2 on 07/19/16 at 1133 the following was observed:
1) One of three boxes 4-0 Silk Suture, expired 01/16
2) One of seven boxes 4-0 Vicryl Coated Suture, expired 07/13
3) Two of two packets 5-0 Vicryl Coated suture, expired 07/14
4) Two of two packets 2-0 Nylon Suture, expired 01/15
C. During an interview with the Chief Nursing Officer at the time of the tour, she confirmed the expired sutures.
