New Deficiency
Based on observation, staff interview and document review conducted on July 7, 2017, it was determined that the facility failed to ensure that medications are stored in secured and locked areas.
Findings include:
Reference: Facility policy states, "Security: 1. All medications will be placed in a secure area. (a) Secure will be defined as either a locking medication room, locking storage cabinet or computer assisted med cabinet (Accudose) ... ."
1. During an observation at 10:47 AM, the following medications were found on top of the anesthesia cart in Operating Room #5, unlocked and unattended:
a. Two syringes labeled "Phenylephrine 100 mcg (microgram)/ml (milliliter) in NSS (Normal Saline Solution) 10 ml."
b. One syringe labeled "Epinephrine 4 mcg/ml in NSS 10 ml."
c. One vial of Succinylcholine.
2. These findings were confirmed by Staff #2, Staff #5, Staff #11 and Staff #10.
