Based on medical record review, the facility did not provide outpatient services that meet the needs of the patients in accordance with acceptable standards of practice in the areas of patient monitoring and transfer for Patient #1.
Findings include:
Review of Patient #1's Emergency Department (ED) record, dated 10/18/13, revealed that the patient was seen for evaluation of a syncopal/near syncopal episode and right ankle fracture after a pain management procedure in the outpatient center.
Review of Patient #1's outpatient record, dated 10/18/13, did not reveal any evidence that the patient was monitored after falling post-procedure.
Review of Patient #1's outpatient record, dated 10/18/13, did not reveal any evidence that ambulance transportation to the ED for evaluation and possible treatment was provided after the patient ' s fall.
