Based on observation and interview, the Hospital's Infection Control program failed to assure staff consistently maintained infection control standards of practices in all areas of the hospital. Findings are as follows:

Per observation on 2/23/16 at 9:15 AM, perioperative staff failed to maintain appropriate infection control intervention to maintain a sanitary environment by not cleaning and disinfecting the floor of blood spills in Operating Room (OR) #5. At the completion of a surgical procedure, a staff member accidentally stepped on a blood spill and tracked blood on the OR floor surface, and also upon exiting OR #5 tracked blood on the floor outside OR #5. Perioperative staff identified the blood contamination on the floor outside the OR and initiated cleaning and disinfecting of the floor. However, the floor surface in OR #5 adjacent to the operating room table remained soiled with blood that had been tracked on staff shoes. Staff failed to clean and disinfect the floor prior to transporting a bed into OR #5 for the purpose of transferring the patient to recovery. Staff were observed wheeling the bed over the contaminated floor surface. The bed wheels made contact with the tracked blood creating the opportunity for cross-contamination of other environmental surfaces during transport of the bed within the hospital. Per interview shortly after the observation, the Surgical Services Nurse Manager confirmed staff failed to adhere to infection control standards of practice by failing to disinfect the floor surface of contaminated blood.
