Based on observation it was determined the facility has locking devices on exit egress doors that do not comply with NFPA 101 Life Safety Code 18.2.2.2.4.

Findings include:

During a tour of the facility with the Director of Plant Operations on 04/14/09 between 9:30 AM and 1:00 PM the following deficiencies were observed with egress doors.

1) On the third and second floor the surveyor found more than one delayed-egress locking device in the egress path. The NFPA 101 18.2.2.2.4 code states "Delayed-egress locks complying with 7.2.1.6.1 shall be permitted, provided that not more than one such device is located in any egress path."

The facility did not meet this code requirement.

2) When tested 6 of 6 the delayed egress locking devices did not meet the NFPA 101 7.2.2.6.1 requirements. The code states "An irreversible process shall release the lock within 30 seconds, if approved by the local fire authority, or wise within 15 seconds upon application of a force to release the latch that shall not be required to exceed 15 lbf nor be required to be continuously applied for more than 3 seconds. The initiation of the release process shall activate an audible signal in the vicinity of the door. Once the door lock has been released by the application of force to the releasing device, re-locking shall be manual means only."

Six of 6 doors tested required more than 15 lbf to initiate the devices, the devices did not go into action after 3 seconds of that force. The devices when activated did not release the locking device within the required 30 seconds.

The facility did not meet this code requirement.

3) The facility has controlled egress doors in the exit path which did not meet the NFPA 101 7.2.1.6.2 code which states "Part C The doors shall be arranged to unlock in the direction of egress from a manual release device located 40 in. to 48 in. vertically above the floor and within 5 feet of the secured doors."

The surveyor observed in the entrance to the critical care area on the second floor, the manual card reader was located 14 feet from the secured doors.

The facility did not meet this code requirement.
