Based on record review and interview the facility failed to make the hospital record of 9 of 9 (Patients #2, #3,#4,#5,#6, #7, #8, #9, and #10) Patients medical records readily available.

Findings included:

Based on a list of medical records requested from the hospital that had not been fulfilled compiled on 01/28/25 by Personnel #4.

The hospital received a request for medical records for Patient #2 from a law firm on 06/24/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient #4 from a law firm on 06/30/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient#7 from the patient on 08/23/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient#10 from the patient on 08/23/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient #5 from a law firm on 09/08/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient #8 from a law firm on 10/01/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient #6 from disability services on 10/09/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient #3 from the patient on 11/14/24. The request has not been fulfilled as of 01/28/25.
The hospital received a request for medical records for Patient #9 from the patient on 12/30/24. The request has not been fulfilled as of 01/28/25.

During an interview on 01/28/25 at 09:50 AM Personnel #4 stated there were 594 request for medical records that had not been fulfilled as of 01/28/25. Personnel #4 stated the facility sold in October of 2023. The facility was to have access to the medical records through the electronic medical records provider for one year. On 05/31/24 the facilities access to the medical records was terminated by the previous owners, three months early. Personnel #4 stated the hospital currently does not have access to the medical records.

The hospital's policy titled Release of Patient Information approved 12/23 reflected..."G. Access to Health Care Information. Information from the Hospital...Within 15 days of the request, the Hospital will comply with a request or notify the patient that the requested information does not exist or cannot be found..."
