The 1950 wing is the original Hospital which now is used mainly for administration areas. it is a two story building of fire resistive construction. The exterior walls are brick, and the interior walls, roof and floors are concrete. The west portion of the second floor and first floor are being remodeled at the time of the inspection, the east portion of the second floor the remodel is complete and occupied. There is a one hour separation between the construction and occupied areas. When remodel is completed the entire area will be 100% automatic sprinkler protected.
The 1969 addition is a one story with a basement (walk out) and is built of protected non-combustible material. Exterior walls are concrete with brick veneer. Interior walls are metal studs with 5/8th inch fire code drywall. The roof has iron joists with metal pan and roofing material on top. The ceiling on the main floor are sheetrock in the patient rooms and one hour rated suspended grid and tile in the corridor. The basement has the cafeteria and maintenance and laundry. The structure is concrete beams and floor between levels.
The 1980 addition is a one story structure of fire resistive construction. Exterior walls are concrete block w/ brick veneer. Interior walls are 8 inch concrete block and roof is pre-stressed concrete panels.
2005 addition is a two story with both stories having walk out at street level. The top story is composed of the entry lounge and clinical areas. the lower level is surgery, ER Boiler room and ex ray areas this area is 100% automatic sprinkler protected.
The 2010 addition is a single story of fire resistive construction interior walls are metal studs with 5/8 inch fire code drywall. Central Storage and supply is located in this portion.

Based on staff interview and record review, the facility did not assure that the fire alarm system is in accordance with NFPA 72 and chapter 9.6.4 of NFPA 101 by ensuring that an approved central station appropriately and continuously monitors the fire alarm and summons an organized fire department upon receipt of a fire alarm transmission. This deficient practice affects all occupants of the building. The facility census is 5 with a capacity of 25. Findings include:
Observation on2-1412 at 3:15 p.m. revealed the facility failed to provide information and/or documentation assuring the facility ' s fire alarm signal is continuously monitored by a central reporting station. Observation confirmed by maintenance staff.
NFPA Standard: If the remote supervising station is at a location other than the public fire service communications center, alarm signals shall be immediately retransmitted to the public fire service communications center. 1999 NFPA 72, 5-4.6.1

Based on observation, record review and interview, the facility failed to maintain and test a complete automatic sprinkler system with weekly, monthly and quarterly inspections and testing. All smoke compartments and all occupants could be affected by the deficient practice. The facility has 25 certified beds and at the time of the survey the census was 5. Findings include:

1. Observation on 2/14/12 revealed that the sprinkler riser was not tagged with quarterly inspection and testing tags. Record review of the facility fire safety components on 2/1/07, revealed that the sprinkler system had not been tested quarterly by trained and qualified person. Maintenance Staff verified that quarterly sprinkler testing is not being done by properly trained person.

NFPA Standard: Requires sprinkler systems to be continuously maintained in proper operating condition and an inspection every quarter of a calendar year. 1998 NFPA 25, 2-2 and 2000 NFPA 101, 4.6.12.1
