Based on three (3) of three (3) staff interviews (Staff #4, Staff #5 and Staff #7) and document review on 9/30/2021, it was determined that the facility failed to ensure its Novel Coronavirus Isolation Policy 2021 was implemented for Environmental Services staff.

Findings include:

Reference: The facility policy titled, "Novel Coronavirus Isolation Policy 2021" states. "...Employees: Prior to each shift, must complete Fit to Work Attestation, which includes the latest CDC guidance regarding COVID-19 symptoms. ..."

1. During a tour of the facility at 10:00 AM, in the presence of Staff #2, the following staff were interviewed:

a. During interview at 10:10 AM, Staff #7 [Environmental Services (EVS) staff] stated that he/she works for an agency and is not screened for COVID-19 upon entering the facility. Staff #7 stated that he/she has a temperature check at times, it depends on who is working at the screening station. Staff #7 stated that he/she does not have access to the facility self attestation screening process for COVID-19.

2. During interview with Staff #4 (EVS supervisor) at 11:35 AM, Staff #4 stated that the facility currently has fifteen (15) agency staff that do not have access to the hospital based self attestation screening for COVID-19 screening. Staff #4 stated the contracted EVS staff should be screened prior to entering the facility.

a. During interview with Staff #5 (EVS director) at 11:35 AM, Staff #5 stated that the EVS agency staff is not screened for COVID-19 if they do not enter through the main lobby or the Emergency Department entrance.

3. The above findings were confirmed with Staff #2, Staff #3 and Staff #5 at 2:30 PM.
