RN SUPERVISION OF NURSING CARE

Based on review of facility documents, medical record (MR), and staff interview (EMP), it was determined the facility failed to follow its established policy for hygiene for MR1

Findings include:

A review of of "Pinnacle Health Hospitals Patient Right Booklet" revealed "Care Delivery ... Receive kind , respectful, safe, quality care delivered by skilled staff..."

A review of "Pinnacle Hospitals Nursing Standards of Care" revealed "I. Title: Unit Standard of Care for Medical / Surgical Patients. II. Goals/Expected Outcomes and Nursing Actions: A. The RN exhibiting caring practices will assess the collected data and identify the patient's characteristics(complexity, stability, participation in care and decision making, resource availability, resiliency, predictability& vulnerability) including educational and discharge planning. 1. Data will be collected: a. Initially within 24 hours of arrival on the unit... b. Ongoing: ... 8) Offer Personal Hygiene (daily). 2. The patient/family/significant other will participate in the a assessment of needs..."

A review of the "Clinical Flowsheet Report" revealed:

The review of the medical record revealed no documentation of hygiene care given to the patient on March 17, 19, and 20, 2015.

An interview conducted on April 2, 2015, with EMP 3 confirmed that there was no documention in the record that the patient received hygiene care.
