Based on random observation, staff interview, and review of facility documents, it was determined the facility failed to ensure that proper hygiene procedures are implemented in accordance with facility policies and procedures.
Finding include:
Facility Policy and Procedure titled, "Hygiene/Grooming: Personal (formerly Dress Code)" (last reviewed 1/15/2020) states, " ... II. RESPONSIBILITY a. All Food and Nutrition Services employees to maintain standards of dress, cleanliness, and grooming as indicated by this policy. ... III. PROCEDURE ... C. Mustaches and beards must be clean and well-trimmed. Any facial hair should be covered with a beard net. ..."
Facility Policy and Procedure titled, "Safety: Infection Control (Food and Nutrition)" (last reviewed 10/25/2022) states, " ...IV. General Instructions ... 2. Education ... c. Personal Hygiene 1) Employees engaged in the preparation of food and other persons who may come in contact with these operations shall use effective hair restraints to prevent the contamination of food. Service personnel shall take steps necessary to keep hair from food and food contact surfaces. ..."
On 7/14/23 at 12:11 PM, during a tour of the kitchen in the presence of S6 (Director of Regulatory), S7 and S8 (System Clinical Nutrition Managers), two employees dressed in kitchen attire, S13 (Chef) and S14 (Line Cook) who both had beards, were observed in the kitchen without a beard net. When questioned, S7 confirmed that both S13 and S14 should have been wearing a beard net.
