Based on observation and staff interviews it was determined the facility failed to ensure proper fire stopping materials had been used to seal openings in rated walls.
The findings include:
On 03/13/13 between 9:00 am and 12:00 pm Observation revealed that non approved spray foam insulation had been used to seal around a pipe in HVAC Room 60A.
An interview with Staff M at the time of discovery and this interview revealed that the facility was unaware of the use of the non approved foam.

Based on observation and staff interviews it was determined the facility failed to ensure proper sprinkler coverage was provided throughout the building
The findings include:
On 03/12/13 between 1:00 pm and 4:00 pm Observation revealed that the skylight area was not properly protected by the sprinkler system.
An interview with Staff M at the time of discovery and this interview revealed that the facility was unaware of the requirement for sprinkler protection for the skylight area.

Based on observation and staff interviews it was determined the facility failed to ensure that the requirements of NFPA 13 were met throughout the building.
The findings include:
On 03/12/13 between 1:00 pm and 4:00 pm Observation revealed that the non required partial sprinkler system in the kitchen area is not installed and maintained properly.
Items noted include:
a.) sprinkler heads are loaded with a buildup of grease and lint.
b.) an escutcheon plate missing on two sprinkler heads.
c.) sprinkler heads installed under the kitchen vent hood shall be listed for this use.
An interview with Staff M at the time of discovery and this interview revealed that the facility was unaware of these sprinkler issues on this non required partial system.

The findings include:
On 03/12/13 between 1:00 pm and 4:00 pm Observation revealed that the kitchen K-Type Extinguishers were not provided with the required instruction placard.
An interview with Staff M at the time of discovery and this interview revealed that the facility was unaware that instruction placards were required.

On 03/12/13 between 1:00 pm and 4:00 pm Observation revealed that the kitchen vent hood filters were not installed properly.
An interview with Staff M at the time of discovery and this interview revealed that the facility was unaware that filters had not been reinstalled properly.
On 03/12/13 between 1:00 pm and 4:00 pm Observation revealed that the vent hood for the grill in the serving line area was not protected by an automatic extinguishment system as required.
