Based on document review and interview, the facility failed to complete a patient transfer form to another facility, for 1 of 20 medical records reviewed (patient #P1).
Findings Include:
1. Review of F1 policy for: "Patient Transfer to another Facility from the Emergency Medical Trauma Center (EMTC)," PolicyStat ID: 6372588, last approved 7/2019, states that an "appropriate transfer" must include the completion of the transfer form.
2. Review of P1'S (MR) indicated patient P1 was treated in the emergency department at F1 on 1/5/2021, and transferred to another facility.
3. On 4/21/2021 at 1440 hours, S1, Regulatory Consultant, confirmed that no transfer form was available for P1's transfer to F2.
4. On 4/21/2021 at 1710 hours, MD1, Medical Director, Emergency Medicine, confirmed that no transfer paperwork was completed on P1, and that it should have been.
