Based on observation and staff interview, the facility failed to install the fire alarm system in accordance with 1999 NFPA 72 2-3.5.1. This deficient practice could effect all patients.

Findings include:

On facility tour between the hours of 11:00 AM and 2:00 PM on 09/29/2015, observation revealed that there was a smoke detector located within 36 inches of the HVAC diffuser in Room F123.

This deficient practice was verified by the Maintenance Supervisor at the time of the inspection.

Based on observation and staff interview, the facility did not properly secure the oxygen cylinders in accordance with 1999 NFPA 99 4-3.5.2.1. This deficient practice could affect all patient.

Findings include:

On facility tour between the hours of 11:00 AM and 2:00 PM on 09/29/2015, it was observed that the "H" Type cylinders in the oxygen room were not individually secured.

This deficient practice was verified by the Maintenance Supervisor at the time of the inspection.
