Based on observation, interview, and record review, the facility failed to maintain two hour barrier between nonconforming buildings, failed to provide construction of a type to accommodate not more than one story in a fully sprinklered building, failed to ensure its corridor doors with latching hardware closed and latched, failed to maintain the rating of the barriers protecting vertical openings, failed to ensure doors in smoke barriers did not completely self close, failed to ensure protective construction in hazardous areas were maintained, failed to ensure a gift shop opening onto cafeteria was less than 500 square feet, failed to maintain its automatic sprinkler system in accordance with National Fire Protection Association 25, failed to maintain the protective construction of the chute room where the trash chute terminates, failed to have an intervening wall between medical gas zone shut off valves and the outlets they serve, failed to maintain a one hour occupancy separation barrier between the ASC and the other occupants of the building, failed to ensure compliance with 19.2.7, National Fire Protection Association 101, 2000 edition, discharge from exits and failed to use power strips in accordance with National Fire Protection Association 70, 1999 edition. (A709) This has the potential to affect all patients receiving services at the facility. The facility had 502 patients at the time of the survey.

LIFE SAFETY FROM FIRE

Based on observation, interview, and record review, the facility failed to meet requirements for life safety, specifically, the applicable provisions of the 2000 edition of the Life Safety Code of the National Fire Protection Association. This has the potential to affect all patients receiving services at the facility.The facility had a census of 502 patients.

Findings include:
