Based on document review and interview, it was determined that the hospital failed to ensure that the ED (emergency department) log included disposition for all patients seeking care in the ED.
Findings include:
1. On 7/09/2024, the hospital's policy titled, "Emergency Medical Treatment and Patient Transfer Policy" (7/2023) was reviewed and included, "... Definitions... Central log means a log of the Hospital maintains of all individuals who present to the Hospital seeking emergency medical assistance and the disposition of such individuals, whether the person refused treatment, was transferred, was admitted and treated..."
2. On 7/10/2024, the hospital's ED logs from February 1, 2024, through July 9, 2024, were reviewed. The ED logs indicated the following:
- On 2/13/2024, there were three patients who presented to the ED with no disposition.
- On 7/02/2024, there were five patients who presented to the ED with no disposition.
- On 7/05/2024, there were two patients who presented to the ED with no disposition.
- On 7/06/2024, there were two patients who presented to the ED with no disposition.
3. On 7/11/2024 at approximately 11:00 AM, findings were discussed with E #1 (ED Director) and E #10 (Director of Risk and Quality). E #1 and E #10 agreed that the ED logs lacked disposition for the patients who presented to the ED. E #10 stated that moving forward, a staff will be assigned to ensure that the ED logs are complete.
