Based on observation during the interior tour the facility failed to maintain smoke barriers free of penetrations, to prevent the passage of smoke, as required by NFPA 101, 8.3.6, and 19.3.7.3.
Findings include:
1. On 02/28/11, the first day of survey, during the interior tour conducted from 1:15 p.m. to 3:30 p.m., accompanied by facility staff, observation revealed multiple 1 to 3 inch penetrations by electrical conduits in which no material was provided to maintain the smoke resistance between the suspended ceiling and the interstitial space above the suspended ceiling in the electrical rooms identified by facility staff as A1, A2, D2, F2 and F3.

Based on observation the facility failed to provide complete sprinkler system coverage for all portions of the building as required by NFPA 13, 5-13.1, and NFPA 101, 19.3.5.
Findings include:
1. On 03/01/11, the second day of survey, during the interior tour of the ground floor from 10:30 a.m. to 2:30 p.m., accompanied by facility staff, observation revealed no sprinkler head installed in the main electrical room and no sprinkler head installed in the corridor alcove identified by facility staff as A1.
