K200 Means of Egress Requirements - General
Findings:
The inspector observed, while accompanied by the Plant Engineering Director, during the hours of the inspection from 10:00 am to 1:15 pm on 04/06/2023, that there were the following issues:
1) Based on review of the records, facility failed to perform inspection of doors in means of egress every year. For example, there was no records for 2022.

K223 Doors with Self-Closing Devices
Findings:
The inspector observed, while accompanied by the Plant Engineering Director, during the hours of the inspection from 10:00 am to 1:15 pm on 04/06/2023, that there were the following issues:
1) Observed during tour, the double door in the medical surgical unit was missing an astragal. Verify throughout facility and provide astragal where missing.

K912 Electrical Systems - Receptacles
Findings:
The inspector observed, while accompanied by the Plant Engineering Director, during the hours of the inspection from 10:00 am to 1:15 pm on 04/06/2023, that there were the following issues:
1) Observed during tour in med/surg room 115, there was a broken electrical outlet that needed replacing.
2) Facility failed to perform yearly testing of hospital-grade receptacle in patient care areas.

Findings:
The inspector observed, while accompanied by the Plant Engineering Director, during the hours of the inspection from 10:00 am to 1:15 pm on 04/06/2023, that there were the following issues:
1) Based on observation of the records, facility failed to perform weekly inspection of the generator.
