K-130 Procedures in Case of Fire

NFPA 101 Life Safety Code, (2000 Edition) 19.7.2.1* For health care occupancies, the proper protection
of patients shall require the prompt and effective response of health care personnel. The basic response required of staff shall include the removal of all occupants directly involved with the fire emergency, transmission of an appropriate fire alarm signal to warn other building occupants and summon staff, confinement of the effects of the fire by closing doors to isolate the fire area, and the relocation of patients as detailed in the health care occupancy ' s fire safety plan.

Based on interview and document review, the facility failed to fully implement the facility fire plan as evidenced by the failure to provide documentation for the activation of the fire alarm during the incident. This had the potential for the delay in fire department notification and staff notification resulting in harm to patients.

Findings:

During the document review on July 28, 2010, the incident documents were reviewed.

At 1:35 p.m., the " Fire Safety Management Plan " was reviewed. On page 5 of the plan under " Fire Plan Elements " the facility plan was to use the acronym " RACE " in the event of a fire and to " Alarm by pulling fire alarm pull stations and calling 2222 on the phones. " The report provided to the California Department of Public Health by the facility failed to document the activation of the fire alarm system. The Security Department Incident Report failed to document the activation of the fire alarm system. The Security Officer documented " I heard the Code Red over the intercom " and responded to the scene. Staff 1, Staff 2 and Staff 3 confirmed the documents failed to document that the fire alarm system was activated during the incident.

At 2:10 p.m., the Assistant Fire Chief for the Barstow Fire Protection District stated the fire alarm was not active when the fire department arrived on scene.
