Based on observation of surveyor 20980 on January 24, 2011:

Pinkham corridor wall has rug attached to wall that once was used to display pictures. Rug is now exposed and there is no documentation of flame spread for rug.

The manager of maintenance and plant engineering was present during the observation.

Based on observation of surveyor 20980 and 17632 on January 24th, 2011:

1) The clean storage room door in the emergency department corridor does not latch. (door repaired on return of surveyor on 01/25/11)

2) Janitors closet in the emergency department corridors does not have a self closing device on door. (self closer on door when surveyor returned on 01/25/11)

The manager of maintenance and plant engineering was present during this observation.

Based on observation of surveyor 20980 on January 25 th, 2011:

3) Storage room door does not have self closing device in labor and delivery area.

Based on observation of surveyor 16732 on January 24, 2011:

Trauma rooms 1,2 and 3 in the emergency room department have sliding locks on patient side of room that could lock staff out of room. (locks were removed immediately)

Based on observation of surveyors 20980 and 16732 on January 24, 2011:

1) Exit sign in office hall area of emergency department area did not have continuous illumination. (The sign was repaired on surveyors return on 01/25/11)

2) Green Ridge Nureo Rehab has signage on exit door that says "this is not an exit" on a requited exit door. (The sign sign was moved immediately)

3)Surgical Services to main Hospital exit corridor has signage that reads "authorized personal only" on door.

Based on observation of surveyor 20980 on January 25, 2011:

Old smoke detectors/heat detectors in exit corridor between laundry wash/dry are not operable and still placed on ceiling.

The lead maintenance coordinator was present during the observation.

Based on observation of surveyors 20980 and 16732 on January 24, 2011:

1) Sprinkler head in procedure room of emergency department is not properly placed. It is within 4 inches of the wall.

2) Sprinkler head obstructed by light fixture in janitors closet in emergency room corridor. (light moved on return of surveyor on 01/25/11)

3) Escutcheon plate missing from sprinkler head in office at DSU/OR public corridor. (plate on when surveyor returned on 01/25/11)

Based on observation of surveyor 20980 on January24th, 2011:

4) PACU storage room in operating room area has shelving too close to sprinkler by entrance obstructing sprinkler

Based on observation of surveyor 20980 on January 25, 2011:

5) Storage room in laboratory has sprinkler near large cooler that is obstructing spray pattern.

The lead maintenance coordinator was present during the observation.

Based on observation of surveyor 16732 on January 25, 2011:

6) There is no extinguishing coverage (sprinkler or chemical) for IS Computer room and no 2 hour fire separation from rest of facility.

Connecting exit corridor from Surgical Services to main Hospital has metal studs laying on the floor obstructing egress. (Studs were removed immediately)

1) Snow Flake decorations hanging from ceiling of OR recovery room with no documentation of flame resistance.

2) Two cubicle curtains in OR Recovery room has no labeling of NFPA 701 testing.

3) Large retirement banner hanging in Pinkham corridor with no documentation of flame resistance

1) There is no labels for piped in gases in the PACU storage room on right of door entrance.

2) The medical gas box is not labeled in OR recovery

Ashland Health Center, 33 Walker St, Ashland

1) Dental Lab area - VacStar 4 Dental vac system in boiler room needs J-box working with wire nuts.
2) HH-AHC/BIO-29 Lamp has brown cord taped to lamp.
3) Power strip with 4 appliances plugged into it being used for permanent use.
4) Elevator machine room shall be 1 hour fire rated and all penetrations out of room in wall shall have equal rating. Several penetrations not properly sealed.
5) Air intake for elevator machine room shall have fire damper in vent pipe. Pipe gets air from mechanical room space and no fire damper in pipe.
6) Basement mechanical room (adjacent to conference room) has storage within 3 feet of electrical panel.
7) 1st floor dirty/bio room has door propped open. There are self closing springs attached to fire door but does not self close and positive latch
8) Exam rooms 4 has kickstand to keep door open and doors are open to exit corridor.
9) Exam room 3 does not latch shut and door is open to exit corridor.

1) Provide emergency and exit signage for area that was not being used when building was occupied but now being used for storage area.
2) Fire extinguisher shall be properly marked.

1) Guardian pull station for suppression system in OT/PT area shall be clearly visible and in the path of egress.

Day surgery recover area has refrigerator that is plugged into a extension cord which is plugged into outlet on wall. (longer cord was put on refrigerator on return of surveyor on January 25, 2011 and power relocatable tap was removed)
