Based upon staff interview and observations made in the presence of the plant manager on 6/18/12, it was determined that the facility did not maintain the fire sprinkler system in accordance with NFPA 25 and NFPA 101 19.3.5.1 & 19.1.6.

Findings include:

1. The fire sprinkler head assembly located in the canopy of the emergency room and the entrance to the Long Term Care wing were observed to be installed in the wrong position. Upright type sprinkler heads were installed in the place of a pendent style sprinkler head, this could affect the way the water is discharged from the system and fail to extinguish a potential fire.

2. The foyer and the entrance corridor to the Long Term Care Center was observed to be lacking any sprinkler protection.
