Based on a tour of Mc Keldin Building it was observed that:

Combustible fiber ceiling tiles remaining within isolated rooms shall be removed or shall be painted with an approved fire retardant paint. (19.3.3)

Based on a tour of Hitchman Building it was observed that:

Patient room doors to isolation-seclusion rooms in each wing lack latching hardware suitable for keeping the doors tightly closed. Magnets which have been installed to keep the doors closed are ineffective. Approved positive latching hardware shall be provided. (19.3.6.3)

Based on a tour of Mc Keldin Building it was observed that:

The door to room #A21 shall be adjusted to positive latch. (19.3.6)

B. The coordinating device on the pair of cross corridor fire barrier doors near room #301 shall be adjusted in order that the doors close properly. In addition, these doors shall be repaired to positive latch. (10.2.2.2)

Based on a tour of Mc Keldin Building it was observed that:

One of the doors in the smoke barrier in the attic was found open. The spring-loaded hinge on this door was in need of repair. This smoke barrier door shall be self-closing and shall be kept closed. (19.3.7)

Based on a tour of Mc Keldin Building it was observed that:

Storage levels in linen storage rooms #C20-B and #C20-C must be reduced. (19.3.2)

Based on a tour of Mc Keldin Building it was observed that:

Patient room doors provide a clear opening of 41.5 inches as required. However, several offices with 36 inch doors have been converted to patient rooms. These spaces may be used as patient rooms only if door widths are increased to provide a clear opening of at least 41.5 inches. (18.2.3.5)

Based on a tour of Hitchman Building it was observed that:

A sign has been placed on one of the exterior exit doors reading "NOT AN EXIT TO THE PARKING LOT". This door is a required exit door and the "NOT AN EXIT" wording shall be changed. (10.2.10.1)

Based on a tour of Hitchman Building it was observed that:

A heat detector in janitors closet #108 is marked "NOT IN SERVICE". This fire alarm device shall be repaired or shall be removed. (19.3.4)

Based on a tour of Mc Keldin Building it was observed that:
Storage was found blocking a fire alarm horn/strobe in room #C22. (19.3.4)

Based on a tour of Hitchman Building it was observed that:

Sidewall sprinklers in the entry lobbies of wings C & D are installed about six feet below the ceiling and do not comply with the positioning provisions of NFPA 13. The arrangement shall be corrected. (10.3.5)

Based on a tour of Mc Keldin Building it was observed that:
Surface mounted light fixtures obstruct automatic sprinkler discharge in various rooms throughout the building. The new institutional style sprinklers are smaller with deflectors much closer to the ceiling than the original sprinklers. Sprinklers shall be moved or lowered or the light fixtures shall be relocated so as to provide adequate sprinkler coverage. (19.3.5)

Based on a tour of Salomon Building it was observed that:
A. Approved automatic sprinkler protection shall be provided in the entry vestibule addition near room #101. (19.3.5)

B. Surface mounted light fixtures obstruct automatic sprinkler discharge from the new institutional sprinklers in several isolated rooms including storage room #20A. (10.3.5)
