INFECTION CONTROL SURVEILLANCE, PREVENTION

A. Based on observation, document review, and interview, it was determined the Hospital failed to ensure visitor screening was conducted, in accordance with its policy. This has the potential to affect all patients, visitors, and employees of the Hospital.

Findings include:

1. On 8/11/2020. between approximately 10:00 AM and 10:10 AM, upon entry into the Hospital, the following deficiencies were identified:
a. A visitor was observed in the lobby. The receptionist took the visitor's temperature; however, did not ask any questions.
b. A staff member exited the Hospital and returned with an outpatient. The receptionist took the outpatient's temperature; however, did not ask any questions.

2. Policy CV-1, COVID-19 Screening (last reviewed/revised by the Hospital 4/15/2020) was reviewed on 8/11/2020 at approximately 11:15 AM. The policy indicated, "All persons entering the facility will have a screening for travel risk... close contact risk, and symptomology..."

3. An interview was conducted with the Chief Executive Officer (E#1) on 8/11/2020 at approximately 10:30 AM. E#1 stated all visitors were to have questions asked and answered, in addition to having their temperature taken.

B. Based on observation and interview, it was determined the Hospital failed to ensure masks were donned appropriately, covering both the mouth and nose. This has the potential to affect all patients, visitors, and employees of the Hospital.

Findings include:

1. On 8/11/2020. between approximately 10:00 AM and 10:10 AM, upon entry into the Hospital, the following deficiencies were identified:
a. The entry way signage indicated masks were to be worn.
b. A housekeeper was observed at the entrance assisting a person into a vehicle, re-entered the Hospital, continued with mopping the lobby floors. A visitor was observed in the lobby. The housekeeper's mask was worn covering the chin, not the mouth and nose.
c. A staff member was observed bringing a discharged patient to the lobby to the visitor that was present. The staff member's mask was not covering the nose.
d. A staff member exited the Hospital and re-entered with an outpatient. The outpatient lacked a mask and was not instructed to apply a mask.

2. An interview was conducted with the Chief Executive Officer (E#1) on 8/11/2020 at approximately 10:30 AM. E#1 stated, "I noticed that when I came up to the lobby (the housekeeper's mask being on the chin) at that time. All staff know that they are to make sure the mask is covering the mouth and nose or it won't help with anything. All visitors are to wear a mask also."
