Based on medical record review and interview, the hospital failed to ensure a sufficient number of nursing staff were available to ensure all patients received their prescribed medications in order to meet the needs of the patients for 1 of 5 (Patient #4) sampled patients.
The findings included"
1. Medical record review for Pt #4 documented a physician's order dated 2/26/11 at 0755 for the patient to receive Vasotec 2.5 milligrams (mgs) intravenously (IV) every 6 hours.
2. Review of the patient's medication administration record (MAR) documented the Vasoctec had been scheduled to be administered at 0400, 1000, 1600 and 2200. Review of the MAR dated 2/27/11 and 2/28/11 revealed no documentation the patient had received the Vasotec at 0400 and on 3/2/11 had not received the Vasotec at 1600. There was no documentation of a physician's order not to administer the Vasotec at those times.
3. During an interview on 4/13/11 at 1:43 PM, the Chief Nursing Officer (CNO) verified the patient had physician orders to receive Vasotec at 0400 on 2/27/11 and 2/28/11 and at 1600 on 3/2/11. The CNO verified the patient did not receive the Vasotec as ordered.
