Based on random observation during the survey walk-through, not all portions of the building fire alarm system are installed in accordance with 19.3.4.
Findings include:
A. During a test of the fire alarm system it was observed that smoke barrier cross corridor doors did not close completely to comply with 7.2.1.9. Example locations observed:
1. Tower Building - "C" Wing - 6th Floor: Cross corridor doors did not close completely due to one door leaf which did not release from the magnetic hold-open position.
2. Tower Building - "D" Wing - 6th Floor: Cross corridor doors did not close completely due to one door leaf which did not release from the magnetic hold-open position.
3. Tower Building - 5th Floor: Main Entry to the Behavioral Health Unit (smoke doors) did not release to self-close during activation of the fire alarm system.
4. Tower Building - "D" Wing - 3rd Floor building separation with South Pavilion: the 1 1/2 fire door did not close to latch completey during fire alarm activation.
B. Smoke detectors were observed that are installed greater than 12" below the ceiling/deck in non-compliance with NFPA 72, 1999, 2-3.4.3.1. Locations observed include:
1. Tower Building "C" Wing, Fourth Floor - Electrical Closet across Foundation Room 413 has a smoke detector that is improperly installed.
Update 09/16/10: the smoke detector was relocated but is not within 12" of the upper deck.

Findings include;
B. During a test of the building fire alarm system conducted on March 9, 2010, locations were observed from which more than two unsynchronized visual notification devices were visible, as prohibited by 4-4.4.2.2. Areas observed were on each floor of the tower building.
