Based on findings from document review and interview, in 1 of 7 surgical patient medical records reviewed the blood Rh factor (rhogam) listed on the patient's (Patient A's) anesthesia record was not correct.
Findings include:
--Per MR review, Patient A was admitted to the hospital on 6/12/14 for neurosurgery. Patient A's Anesthesia record dated 6/13/14 indicates his blood type (ABO) and Rh factor is B+. However, per review of Patient A's Type and Crossmatch laboratory report dated 6/12/14 his blood type (ABO/Rh) was B-.
--During interview with the Director of Quality Improvement on 1/2/15 this finding was shared.
