Based on document review and interview, the facility failed to ensure a nutrition assessment or means of nutrition in 1 of 10 patient medical records reviewed (patient 6).
1. Review of policy titled, "Nutrition Care Dynamics - Nutrition Referral/Assessment Process", last reviewed 10/2022, indicated that nutritional assessments should be completed on patients whose diet orders are NPO, Clear Liquid, Full Liquid greater than 3 days to ensure appropriate nutrition intervention and care and that follow-up is initiated.
2. Review of patient 6's medical record lacked documentation of a nutrition assessment or nutrition from 12/26/2023 to 01/03/2024. Patient 6 had a diet order written on 12/26/2023 for nothing by mouth (NPO). Patient remained NPO until discharge on 01/03/2024.
3. Interview with A5 (Registered Dietitian), on 01/30/2024 at 2:50 p.m. confirmed that patient 6 met the criteria for a nutrition consult and assessment due to diet orders and patient did not receive a nutrition consult or assessment during their entire hospitalization.
