The facility failed to ensure exit access was readily accessible at all times.

Observation determined:

1) The facility had not ensured that doors in the means of egress were not locked against egress when the building was occupied.

The second floor west stairway door was equipped with an access-controlled magnetic lock that required a code to be entered on a key pad to release.

The deficiency affected egress from one (1) of four (4) exits from the second floor.

2) During its swing, any door in a means of egress shall leave not less than one-half of the required width of an aisle, corridor, passageway, or landing unobstructed and shall not project more than 7 inches into the required width of an aisle, corridor, passageway, or landing, when fully open. 7.2.1.4.4

The corridor door to the Restoration Aid Room opened outward into the corridor. The automatic door-closure device restricted the door from fully opening. The door extended three feet into the exit corridor when opened.

This deficiency affected one (1) of numerous doors in the facility.

Automatic fire sprinkler systems must be installed in accordance with NFPA 13.

Ordinary-temperature-rated sprinklers shall be used throughout buildings. NFPA 13, Section 5-3.1.5

The facility failed to provide ordinary-temperature-rated sprinklers throughout the building.

Observation determined an intermediate-temperature-rated sprinkler was located in the northwest corner of the Kitchen.

Failure to install the automatic sprinkler system in accordance with NFPA 13 increases the risk of injury and death due to fire.

The deficiency affected one (1) of numerous locations protected by the automatic sprinkler system, which serves the entire facility.
