Based on policy and procedure review, clinical record review, restraints log review and interview, it was determined that two (#4 and #6) of three (#4, #5 and #6) patients did not have daily orders for restraints. Failure to follow facility policy of daily orders for restraints for non-violent (medical) affected these two patients and had the likelihood to affect all restrained patients at the facility. Findings follow:
A. Review of policy and procedure titled "Restraints and Seclusion" showed that for non-violent (medical) restraint usage orders must be placed daily.
B. Review of medical chart for Patient #4 from restraint log showed one day the patient was restrained without a daily order from the physician.
C. Review of medical chart for Patient #6 from restraint log showed one day the patient was restrained without a daily order from the physician.
D. The findings of A, B and C were confirmed in an interview with the Vice-President of Nursing on 8/14/2023 at 1:40 PM.
