Based on review of hospital documents, and staff interviews, the Department determined that the hospital failed to require the Cath Lab's medication refrigerator temperatures were checked and recorded daily when the Cath Lab was used, according to policy.

Findings include:

The Cath Lab Refrigeration Temperature Check list, requires: "...Check refrigerator temperature daily and record temperature reading...."

The Cath Lab Refrigeration Temperature check lists dated 06/01/13 through 08/15/13 revealed that temperatures were not recorded daily when the Cath Lab was in use, according to policy, for 5 days in 06/13, 9 days in 07/13, and 4 days in 08/13 (as of 08/15/13).

The Cath Lab Director confirmed during an interview conducted on 08/15/13, that the medication refrigerator temperatures were not checked and documented, according to policy.
