Sprinkler Requirements for Major Rehabilitati

The inspector observed, while accompanied by the CEO, Safety Officer, and the Maintenance Personnel, during the hours of the inspection from 02:30 PM to 06:30 PM on 08/07/2019that there were the following issues. They were the following issues:

A) The sprinkler system was not provided at the electrical room inspected. Please verify within the hospital a sprinkler head is provided in each room.

The inspector observed, while accompanied by the CEO, Safety Officer, and the Maintenance Personnel during the hours of the inspection from 02:30 PM to 06:30 PM on 08/07/2019that there were the following issues. They were the following issues:

A.) The hold open device / door stop is utilized at the fire rated ATS room and Boiler room. Please remove the device or the device released upon the fire alarm activated This door shall remained closed at all time.



B.) Also, there were hold open device / door stop were utilized at the smoke partition egress corridors and the atrium doors. Please remove the device or the device released upon the fire alarm activated This door shall remained closed at all time.

Electrical Systems - Other

The inspector observed, while accompanied by the CEO, Safety Officer, and the Maintenance Personnel during the hours of the inspection from 02:30 PM to 06:30 PM on 08/07/2019that there were the following issues. They were the following issues.

(A) There was a missing receptacle powered from life safety branch and the battery powered operated light fixture at the essential automatic transfer switch location. There was a missing battery powered operated light unit at the essential automatic transfer switch location.



(B) There were flexible conduits utilized from emergency electrical life safety panel. This wiring system shall be a nonflexible metal raceway.



(C) There is not a permanent label and marked red for the FACP breakers at the life safety branch electrical panel.

Gas Equipment - Cylinder and Container Storag

The inspector observed, while accompanied by the CEO, Safety Officer, and the Maintenance Personnel during the hours of the inspection from 02:30 PM to 06:30 PM on 08/07/2019that there were the following issues. They were the following issues.


A) There is no permanent signage provided for medical gas storage room door. This does not comply with NFPA 99, 2002: 5.1.3.1.5. The door shall have the following label:

NO Smoking or Open Flame

Where nitrous oxide is used, the medical gas closet shall have a door labeled: Re. NFPA 99, 2002: 5.1.3.1.5. The door shall have the following label:
