Based on observations, the facility failed to provide readily accessible exit access as defined by NFPA 101 section 7.2.1.5.10.1. The deficient practice affected the entire facility on the day of the survey.


Findings Include:

On May 29, 2018 at 3:15 PM, observation revealed locking devices above 48 inches from the finished floor on the corridor doors to Room 449, Room 453, Supply Room and Therapy Area at the West Campus Building of the facility.

The administrator was made aware of the problem during the exit conference.

Means of Egress - General

Based on observation and interviews, the facility failed to properly maintain exit egress as per NFPA 101 section 19.2.2.2.6. The deficient practice affected the entire facility on the day of the survey.

Findings include:

On May 30, 2018 at 10:40 AM, observation installed lock on the smoke barrier doors near Labor and Delivery Unit of the facility. The lock on the smoke barrier doors had the potential to block the exit egress to and from the Labor and Delivery Unit of the facility.

The administrator was made aware of the problem during the exit conference.

Corridor - Doors

Based on observation and testing, the facility failed to properly protect corridor openings as directed by NFPA 101section 19.3.6.3.5. The deficient practice affected the entire facility on the day of the survey.


Findings Include:

Observations made from May 29 to 30 2018 revealed the following corridor doors in the facility lacked positive latching devices:

1. Radiology Room door on the West Campus facility
2. Break Room door near 2 South hallway on the Main Campus

The administrator was made aware of the problem during the exit conference.
