Based on Temperature and Humidity Log review and interview, it was determined the humidity levels in two of two operating rooms in the facility was regularly maintained below 35% from December 2009 to March 2010; the facility did not take corrective actions. The failed practice had the potential to affect 4 of 4 surgical patients on the day of the survey and all patients admitted for surgery. The findings follow:
A. Review of the Temperature and Humidity Log on 04/05/10 at 1000 revealed humidity levels below 35% were recorded for each operating room as follows:
1) In Operating Room #604, humidity levels below 35% were recorded for 65 of 130 days from December 2009 to March 2010.
2) In Operating Room #605, humidity levels below 35% were recorded for 68 of 130 days from December 2009 to March 2010.
B. In an interview conducted on 04/06/10 at 1515, the Maintenance Director stated he was aware of low humidity levels in the operating rooms, but there were no work orders to correct the problem available for review.
