Based on record review and interview the facility failed to have a 1-hour face-to-face after the initiation of the intervetion on 1 of 1 (Patient #4) patients that were emergently medicated for imminent risk of self-injurious behavor.
Findings included:
Patient #4 was admitted with depression and increased aggression. Patient #4 has a history of Autism. ON 01/01/22 at 0926 Patient #4 was emergently medicated with Haldol 5mg, Benadryl 25mg and Ativan 1mg intramuscularly. There is no 1-hour face-to-face documented on the chart.
During an interview on 04/14/22 ending at 1700 Personnel #1 verified there was not a 1-hour face-to-face documented on the chart.
