Based on observations and staff interview, it was determined that the facility failed to properly maintain the sprinkler system as required as evidenced by the multiple missing escutcheons throughout and the dirty sprinkler heads. These deficiencies could affect all occupants of the facility in case of a fire or other emergency.
Findings include:
During the Life Safety Survey tour of the facility at 12:25 pm on November 1, 2012 with the Maintenance Director, it was noticed that there was a missing escutcheon in the 3rd Floor - Janitor Closet. This deficiency was noticed again at 2:37 pm on November 1, 2012 in the 2nd Floor - Janitor Closet and at 3:25 pm on November 1, 2012 in the Emergency Room Bed #9. Escutcheons were installed during survey.
During the Staff Interview at 2:26 pm, 2:38 pm, and 3:26 pm on November 1, 2012, the Maintenance Director acknowledged the missing escutcheons. He ordered the installation of escutcheons during survey.
During the Life Safety Survey tour of the facility at 3:15 pm on November 1, 2012 with the Maintenance Director, it was noticed that there were several dirty sprinkler heads in the Cafeteria.
During the Staff Interview at 3:16 pm on November 1, 2012, the Maintenance Director stated the dirty sprinkler heads would be cleaned immediately.

Based on observations and staff interview, it was determined that the facility failed to properly maintain electrical wiring and equipment as required as evidenced by a refrigerator connected to surge protector extension cord and exposed wires in the 3rd floor - Electrical Room. These deficiencies could affect all occupants of the facility in case of a fire or other emergency.
Findings include:
During the Life Safety Survey tour of the facility at 11:55 am on November 1, 2012 with the Maintenance Director, it was noticed that a refrigerator was connected to a surge protector extension cord in Room 407 - Director of Education Office. The surge protector extension cords was removed during survey.
During the Staff Interview at 11:56 am on November 1, 2012, the Maintenance Director stated that the refrigerator will be connected directly to an outlet as required. He removed the surge protector extension cord during survey.
During the Life Safety Survey tour of the facility at 12:30 pm on November 1, 2012 with the Maintenance Director, it was noticed that there were exposed wires in the junction box in the 3rd Floor - Electrical Room. An electrical cover was installed in the junction box during survey.
During the Staff Interview at 12:31 pm on November 1, 2012, the Maintenance Director acknowledged the exposed wires in the junction box in the 3rd Floor - Electrical Room. He installed a an electrical cover in the junction box during survey.
