RN SUPERVISION OF NURSING CARE

Based on document review and interview, Nursing Administration failed to provide daily hygiene to 1 of 14 patients (Patient 1).

1. Review of policy titled: Patient Personal Hygiene: Guidelines for Providing Care (CHG Bathing) last approved 08/2018, indicated that personal hygiene will be offered to all inpatients daily.

2. Review of Patient 1's MR indicated on the Nursing Assessment, Hygiene lacked documentation of any hygiene care given on 3 days (02/25/19, 03/02/19 and 03/05/19).

3. Interview on 03/21/19 at 11:30 am with P53 Quality, confirmed lack of documentation of Pt1 getting complete bath, partial bath, oral care, peri-care or refusal on 02/25/19, 03/02/19 and 03/05/19.

ADMINISTRATION OF DRUGS

Based on document review and interview, Nursing Administration failed to provide medications when scheduled for 1 of 14 patients (Patient 1).

1. Review of policy titled: Drug Administration Times last approved 10/2018, indicated non-time critical medications may be given 1 hour before or after the scheduled dosing time for a total window that does not exceed 2 hours.

2. Review of Patient 1's Medication Administration Status Report from 01/18/19 to 02/25/19 indicated there were 169 occasions when medication was given late including, but not limited to, Lactulose liquid, Omeprazole liquid, and Triamcinolone cream.

3. Interview on 03/21/19 at 11:58 am with P52 Quality, confirmed many medications given late with no documented reason.
